Allergy Emergency Plan
If your child has a documented food allergy, or an allergy to any substance that requires medication, an Allergy Emergency Plan must be completed by his/her physician. Please submit the following to the School Nurse before the first day of school:
- Allergy Emergency Plan (found under “Medical Forms” on the parent Forms page)
- Labeled medication in their original containers.
Asthma Treatment Plan
If your child has a documented diagnosis of asthma, an Asthma Treatment Plan must be completed by his/her physician. Please submit the following to the School Nurse before the first day of school:
- Asthma Treatment Plan (found under “Medical Forms” on the parent Forms page)
- Labeled medication in their original containers.
Physical Exam Form
Effective immediately, all students attending SSPJ will now be required to provide a copy of their yearly physicals with an updated copy of all immunizations received.
The following is a list of all mandatory immunizations.
For Pre-K:
- Haemophilus B (HIB) —two doses with one dose on or after 1st birthday
- Pneumococcal Conjugate —two doses with one dose on or after 1st birthday
- DTaP — four doses
- Polio — three doses
- MMR — one dose on or after 1st birthday
- Varicella (Chicken Pox) — one dose on or after 1st birthday
- Influenza must be administered between Sept. 1st – Dec. 31st of the current school year.
For Kindergarten:
- MMR ( Measles, Mumps, Rubella) — two doses or one each of live
- Rubella and live Mumps containing vaccine and two doses of live
- Measles containing vaccine (lab evidence of immunity is also acceptable)
- Varicella — One dose on or after 1st birthday (physician or parental statement of previous varicella, aka chickenpox, the disease is also acceptable)
- DTaP (Diphtheria, Tetanus, Pertussis) — 4/5 doses: four doses with one given on or after 4th birthday or any five doses
- Polio — 3/4 doses: three doses with one dose given on or after 4th birthday or any four doses
- Hepatitis B — three doses or two doses of adolescent formulation between ages of 11-15 if not previously vaccinated
For Sixth Grade Students who have reached age 11 or within two weeks after they reach age 11.
- Tdap ___ one dose
- Meningococcal Conjugate ___ one dose
All Sixth through Eighth Grade students will also need to provide a copy of the PPE Medical Eligibility Form along with their physical. This form must be on file in the Health Office for all students before the start of school and /or the first day of practice for a sports team if the child is participating in a sports program.
Flu Shots
Flu Shots are required annually for all students under 5-years-old. Please submit a note or updated immunization record from the child’s physician to the School Nurse.
Immunizations
Saints Philip and James School abides by the New Jersey Department of Health Vaccine Preventable Disease Program. During the summer, please contact your child’s doctor to see if your child is up-to-date on his/her vaccines.
Please note the following:
- Children under 5-years-old: Flu shot is required annually.
- 6th Grade Students (or 11-years-old): TDAP and Meningococcal vaccines are required.
